Nutra-IQ: Bringing Structured Nutritional Assessment into the Core of Cancer Care

Nutra-IQ

Health Voitho Technologies Pvt. Ltd. is the exclusive technology partner of Esperer Onco Nutrition in building Nutra-IQ, an AI-supported clinical protocol developed to close one of oncology's most under-recognized gaps: the systematic detection and management of malnutrition and cancer cachexia.

The Problem:A Complication Hiding in Plain Sight

Malnutrition and cancer cachexia are among the most under-recognized, under-diagnosed, and under-treated complications of cancer, in India and globally. Between one-fifth and two-thirds of cancer patients present with malnutrition depending on cancer type, stage, and setting. Indian hospital-based studies report severe malnutrition in over half of treated patients, figures consistently higher than in high-income health systems due to late-stage presentation and pre-existing nutritional vulnerability.

This isn't a passive marker of advanced disease. Malnutrition independently worsens chemotherapy tolerance, increases treatment interruptions and infection risk, prolongs hospitalization, and is estimated to contribute directly to 10–20% of all cancer deaths, separate from the malignancy itself.

Yet nutritional assessment in routine oncology practice remains largely manual, subjective, single-time-point, and disconnected from the biochemical and fluid-status data already being generated during cancer treatment. Validated tools like PG-SGA, NRS-2002, and GLIM are clinically sound, but none were designed to computationally integrate anthropometric, biochemical, inflammatory, functional, and fluid-status data into a single, dynamically updated output. None carry systematic validation in Indian oncology populations, whose dietary patterns, disease-stage distribution, and comorbidity profile differ meaningfully from the cohorts they were built on.

Malnutrition in Perspective

20%–67%

of cancer patients present with malnutrition, depending on cancer type, stage, and setting

>50%

severe malnutrition reported among treated patients in Indian hospital-based studies

10–20%

of all cancer deaths are estimated to be directly attributable to malnutrition

The Solution:Nutra-IQ

Nutra-IQ, The Assessment Protocol, is Esperer Onco Nutrition's response to this gap: an AI-supported computational protocol that:

Captures multi-domain data

Spanning anthropometric, biochemical, inflammatory, functional, and fluid-status measures

Corrects for fluid status

Adjusting anthropometric indices for the way edema, effusions, and hypoalbuminemia mask true lean-mass depletion in advanced disease

Computes nutritional risk

Producing a composite nutritional risk score and staging the patient along the cachexia continuum

Generates a care plan

Auto-generating a tiered, individualized nutrition care plan with a built-in monitoring and re-assessment schedule

The protocol has been applied and refined across a growing base of Indian oncology cases, including complex metastatic and palliative-stage patients, giving it a validation base directly relevant to the population it's built to serve.

Why Positioning Matters

Nutritional assessment in oncology is too often treated as a supportive, secondary activity, offered once symptoms are already severe, rather than run as a structured, repeated evaluation alongside oncological staging and treatment planning. Nutra-IQ is designed to sit differently: not as a wellness or lifestyle add-on, but as a clinical decision-support layer embedded in the core oncology care pathway, from diagnosis through active treatment, survivorship, and palliative care.

The clinical rationale is threefold:

01

Malnutrition is an independent prognostic factor

Patients malnourished at first oncology visit show measurably worse chemotherapy tolerance, higher rates of dose reduction and treatment delay, higher complication rates, longer hospital stays, and reduced survival, consistently shown across the NUTRIONCO study and related cohorts.

02

Nutritional decline is often clinically silent

Body weight, the most common bedside proxy for nutritional status, is systematically unreliable in advanced disease because fluid retention, edema, effusions, and hypoalbuminemia can inflate or stabilize recorded weight even as true lean body mass and skeletal muscle continue to deteriorate.

03

The window for intervention closes quickly

Cachexia progresses along a continuum, from pre-cachexia through cachexia to refractory cachexia, and later stages respond progressively less to nutritional intervention of any kind. Late detection doesn't just delay benefit; it removes it.

This is why Nutra-IQ is designed to function as a repeated, longitudinal clinical measurement, analogous to vital signs or tumor markers, rather than a one-time intake questionnaire: embedded at diagnosis, reassessed at defined treatment milestones, and capable of flagging deterioration before it becomes irreversible.

Health Voitho's Role:The Technology Behind Nutra-IQ

As the exclusive technology partner of Esperer Onco Nutrition, Health Voitho is responsible for building the technology that makes Nutra-IQ's structured, longitudinal nutritional assessment possible at the point of care, from the computational protocol itself to its integration into the oncology care pathway. This work reflects Health Voitho's broader mission: building the technology infrastructure behind precision, preventive healthcare.
